Role Responsibilities:
The Clinical Care Coordinator will oversee the patient care journey, ensuring seamless coordination with physician offices to facilitate appropriate screenings for patients facing high-risk medical and functional challenges. This role includes:

  • Scheduling specialist appointments to optimize patient readiness prior to surgical procedures.
  • Completing patient-reported outcome forms and ensuring the thoroughness of pre-operative health assessments.
  • Coordinating follow-up appointments and assisting in the development of post-operative discharge plans.
  • Identifying high-risk patients across medical, functional, and social dimensions to arrange necessary resources and adhere to clinical pathways, thereby enhancing patient outcomes before surgical interventions.
  • Participating in the creation and delivery of pre-operative educational sessions, utilizing various formats (in-person, online, printed materials) to ensure patients are well-informed.
  • Educating patients on any remote monitoring systems utilized in their care.

Patient Advocacy:
The Clinical Care Coordinator will act as a liaison among physicians, healthcare staff, and patients to guarantee a successful care experience. This includes:

  • Following up with patients and initiating annual reviews as necessary to complete patient-reported outcome measures.
  • Assisting in the assessment of patient needs at discharge and coordinating necessary services, such as home health care and outpatient physical therapy.

Quality Improvement:
Engagement in the review of quality metrics is essential to maximize outcomes, particularly concerning discharge dispositions and readmission rates.

Staff Development:
Participation in staff education sessions, whether individually or in groups, is required. The Clinical Care Coordinator will keep the nurse manager informed of any changes that may affect care delivery and maintain regulatory knowledge pertinent to the specialty area at all levels.

Qualifications:
Required Minimum Education: Bachelor’s Degree

Preferred Education:
Graduate Degree

Experience:
A minimum of two years of relevant clinical experience is necessary.
