Career Opportunity:
We are seeking an experienced Registered Nurse – On Call to join our team. As a key member of our interdisciplinary group, you will be responsible for delivering high-quality patient care services, collaborating with healthcare professionals, and maintaining accurate clinical records.
Key Responsibilities:
• Provide direct patient care services, focusing on the unique needs of each individual
• Act as a liaison between physicians, patients, and families, ensuring seamless communication
• Manage an interdisciplinary team, enhancing the quality of care provided
• Evaluate patient needs and develop personalized plans of care
• Maintain accurate and up-to-date clinical records
• Participate in company-wide quality assessment and performance improvement initiatives
Your Qualifications:
To succeed in this role, you should possess the following qualifications:
• A current RN license (in the state of employment or ability to obtain)
• 1+ years of experience in hospice, home health, or oncology preferred
• Compassionate and empathetic approach to patient care
