Expert Hospice Care Professional

About the Role:
We are seeking an experienced Registered Nurse to join our team as a compassionate patient advocate. As a key member of our interdisciplinary care team, you will provide direct patient care services to hospice patients and their families after normal business hours on an on-call basis.
Key Responsibilities:
- Provide high-quality patient care services, including evaluating patient needs and assisting in the preparation and execution of individualized plans of care.
- Act as a primary liaison between physicians, patients, and their families, ensuring seamless communication and coordination of care.
- Maintain accurate and up-to-date patient clinical records, adhering to all relevant laws and regulations.
- Participate in quality assessment and performance improvement initiatives, driving continuous quality enhancement.
About You:
To succeed in this role, you must be a compassionate, kind, and empathetic individual with a current RN license (in the state of requested employment or ability to obtain). A minimum of 1 year of nursing experience in hospice, home health, or oncology is preferred, but not required.
