About the Role

We are seeking a dedicated and compassionate Nurse Care Specialist to provide high-quality patient care in our Altoona, PA office.

Job Responsibilities

As a Nurse Care Specialist, you will work closely with patients, families, and healthcare teams to deliver comprehensive nursing care. Your primary focus will be on coordinating total plans of care, achieving continuity of patient care, and ensuring timely documentation.

You will perform initial and comprehensive nursing evaluations, develop and implement personalized care plans, and collaborate with physicians to ensure accurate and complete documentation. Additionally, you will coordinate with agency personnel, patients, family members, and community resources to provide seamless care.

Key Performance Indicators:

  • Ensure timely and accurate completion of patient assessments and care plans.
  • Collaborate effectively with healthcare teams to achieve optimal patient outcomes.
  • Maintain up-to-date knowledge of medical necessity guidelines and payer sources.
Requirements

To succeed in this role, you must possess:

  • A current RN licensure in PA.
  • A current CPR certification.
  • A valid driver's license, vehicle insurance, and reliable transportation or access to public transportation.
About the Opportunity
