About the Role

We are seeking an experienced Palliative Care Specialist to join our team at Hospice of the Western Reserve. As a key member of our interdisciplinary team, you will play a critical role in providing compassionate and high-quality care to patients and families facing serious illnesses.

Job Summary

The Palliative Care Specialist will be responsible for facilitating HWR's efforts to relieve suffering, enhance comfort, promote quality of life, and foster choice in end-of-life care for all patients and families. This position requires a strong clinical background, excellent communication skills, and a passion for palliative care.

Responsibilities
  • Clinical Practice: Develop and implement new programs that affect clinical practice, perform holistic patient assessments, prescribe for symptom management, and coordinate care with team members and healthcare professionals involved in patient and family plans of care.
  • Consultation: Ensure quality care across the continuum as an expert in palliative care, facilitate and/or participate in transdisciplinary team conferences, serve as a consultant to staff caring for patients in hospitals, long-term care settings, and home settings, and serve as an expert consultant to healthcare professionals at the local, national, and international levels to influence and advance clinical practice in palliative care.
  • Education: Maintain a thorough knowledge of current practice and literature relating to palliative care, educate patients and families on applicable areas of palliative care, empower patients and families to identify and meet goals of care, provide education to HWR staff in areas of palliative care, and provide education to outside agencies, hospitals, and long-term care settings on principles of palliative care.
  • Collaboration: Maintain a standard care agreement with HWR Medical Directors, work as part of a team, collaborate with members of the palliative care team, collaborate with healthcare professionals as appropriate within the community, facilitate communication between nursing staff/physicians and patient/family, participate on HWR committees, participate in quality improvement activities, maintain and cultivate professional relationships in healthcare that strengthen HWR's strategic marketing plan, and fulfill professional growth through involvement in professional organizations, presentations, and publications.
Requirements
  • Maintain a current Registered Nurse license in the state of Ohio
  • Maintain a current Certificate of Authority in the state of Ohio
  • Master of Nursing Degree
  • Minimum of two years in clinical setting
  • Must be flexible and have problem-solving skills, prioritize, and demonstrate healthy coping strategies
  • Ability to demonstrate excellence in written and oral communication
  • Ability to meet patient/family needs that require variable hours
  • Knowledge of hospice and palliative care clinical standards of practice, policies, and procedures
  • Ability to practice in accordance with HWR clinical standards of practice, policies, and procedures
  • Ability to establish and maintain collaborative working relationships with HWR employees, community, and healthcare providers
  • Ability to provide own transportation
  • Must have and maintain a valid driver's license, good driving record, and auto accident insurance at a level acceptable to HWR and/or HWR's insurer
Compensation and Benefits

We offer a competitive salary range of $80,000 - $110,000 per year, depending on experience, plus a comprehensive benefits package, including health, dental, vision, retirement, and voluntary benefits, as well as tuition assistance, technology package, and protocols in place for wellbeing during COVID-19.
