About the Role:

University Hospitals is seeking an experienced Advanced Practice Provider to join our Palliative Care team at UH Cleveland Medical Center. As a key member of our interdisciplinary team, you will provide compassionate and high-quality care to patients and their families.

Responsibilities:

  • Assess patient needs and develop individualized plans of care.
  • Collaborate with healthcare providers to ensure seamless patient care.
  • Order and perform diagnostic procedures as needed.
  • Develop and implement treatment plans in accordance with established protocols.
  • Maintain accurate and detailed patient records.
  • Contribute to the education and development of staff members.
  • Participate in quality improvement initiatives to enhance patient outcomes.

Requirements:

  • Demonstrated competence as a practitioner.
  • Effective oral and written communication skills.
  • Technical patient care devices specific to the clinical area.
  • Computers and selected office equipment.

Compensation and Benefits:

  • $120,000 - $150,000 per year, depending on experience.
