Hospice of the Western Reserve is seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Care Specialist. This role involves providing high-quality care to terminally ill patients in various settings, including community-based teams, inpatient units, and clinical admissions.

Job Description
  • Practice according to HWR's clinical standards, policies, and procedures
  • Assess patient and family needs, provide intermittent skilled nursing services, and manage pain and symptoms
  • Collaborate with physicians, healthcare professionals, and volunteers to ensure seamless care
  • Develop and implement individualized care plans, educate patients and families on health management, and facilitate effective grieving processes
Required Skills and Qualifications
  • Licensed RN in the State of Ohio
  • Experience in hospice care, home health, oncology, and/or pain and symptom management preferred
  • CPR certification required; Hospice and Palliative Care Certification desired within two years of hire
  • Valid driver's license, good driving record, and auto insurance with adequate coverage

Estimated Salary Range: $70,000-$90,000 per year
