Care Management Support Specialist

The Sparc Network is in search of a dedicated Care Management Support Specialist to enhance our Care Management Team. Our organization is committed to the vision of keeping individuals out of institutional care while providing essential behavioral health services to a diverse population.
We are looking for passionate Care Management Support Specialists for our Tailored Care Management Program. These roles can be based remotely, offering flexibility to our team members.
Qualifications for a Care Management Support Specialist:- Must be at least 18 years of age.
- High school diploma or equivalent is required.
- Must meet one of the following criteria:
- 1) Personal experience with an intellectual/developmental disability (I/DD) or traumatic brain injury (TBI) and knowledge of the North Carolina Medicaid system.
- 2) Experience as a Certified Peer Support Specialist with a behavioral health condition.
- 3) A parent or guardian of someone with an I/DD, TBI, or behavioral health condition, with at least two years of experience navigating the Medicaid system on their behalf (note: parents/guardians cannot serve as extenders for their own family members).
- 4) Two years of paid experience in relevant functions, including at least one year working directly with the Tailored Care Management eligible population.
Individuals with the following experiences may qualify for this position:
- Certified Peer Support Specialists
- Community Health Workers (CHW) who have completed the NC Community Health Worker Standardized Core Competency Training
- Former Community Navigators prior to Tailored Plans implementation
- Family Navigators as defined by Trillium Health Resources
- Parents or guardians of individuals with I/DD, TBI, or behavioral health conditions (not for their own family members)
- Individuals with lived experience related to I/DD, TBI, or behavioral health conditions
Care Management Support Specialists will assist care managers in delivering Tailored Care Management through various activities, including:
- Conducting outreach and follow-up with members.
- Coordinating services and appointments, including reminders and transportation arrangements.
- Engaging in health promotion and sharing knowledge.
- Providing updates to care managers about members' circumstances.
- Tracking referrals and assisting in accessing community resources.
- Participating in case conferences.
- Supporting care managers in assessing unmet health-related needs.
