Physician Assistant

Pine Park Health is seeking a highly skilled Physician Assistant to join our team of healthcare professionals. As a Geriatric Care Specialist, you will be responsible for providing high-quality, comprehensive in-home healthcare services to seniors in Northern Nevada.
Key Responsibilities- Diagnostic Evaluation and Management: Conduct diagnostic evaluations, manage chronic conditions, and provide therapeutic healthcare to primary care patients residing in assisted living communities, residential care homes, and private residences.
- Health Assessments: Perform annual comprehensive health assessments in accordance with health plan partners and Medicare guidelines, including conducting health assessments, reviewing past medical history, and identifying care gaps.
- Care Coordination: Coordinate with other members of the care team, including internal and external partners, to ensure seamless and convenient care for patients.
- Documentation and Communication: Accurately and timely document patient information in the Electronic Health Record (EHR) and communicate effectively with patients, families, and healthcare teams.
- Licensure and Certification: Current Nevada PA or APRN license, credentialed with Medicare, Medicaid, and health plans Pine Park Health is actively contracted with.
- Experience: Minimum two years of experience in specialty care area (Geriatrics) in private physician office, clinic setting, or community-based program.
- Clinical Competence: Demonstrates clinical competence, including strong communication skills and medical knowledge.
