At UnitedHealth Group, we're committed to making healthcare work better for everyone. As a Nurse Practitioner or Physician Assistant on our Home-Based Medical Care team, you'll play a critical role in delivering high-quality, patient-centered care to our most vulnerable members.

Key Responsibilities:

  • Conduct comprehensive assessments and develop personalized care plans for patients
  • Collaborate with interdisciplinary teams to coordinate care and address patient needs
  • Prescribe diagnostics and interventions to prevent unnecessary hospitalizations
  • Provide education and support to patients and their caregivers
  • Participate in quality improvement initiatives to enhance patient outcomes

Requirements:

  • Completion of Nurse Practitioner or Physician Assistant accredited program
  • Current state RN license and NP/PA license
  • Access to reliable transportation

Preferred Qualifications:

  • 2 years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care, community resources, and cost-effective alternatives
  • Knowledge of CPT, ICD-10, and HCPC codes
