Nurse Practitioner or Physician Assistant

At UnitedHealth Group, we're committed to making healthcare work better for everyone. As a Nurse Practitioner or Physician Assistant on our Home-Based Medical Care team, you'll play a critical role in delivering high-quality, patient-centered care to our most vulnerable members.
Key Responsibilities:
- Conduct comprehensive assessments and develop personalized care plans for patients
- Collaborate with interdisciplinary teams to coordinate care and address patient needs
- Prescribe diagnostics and interventions to prevent unnecessary hospitalizations
- Provide education and support to patients and their caregivers
- Participate in quality improvement initiatives to enhance patient outcomes
Requirements:
- Completion of Nurse Practitioner or Physician Assistant accredited program
- Current state RN license and NP/PA license
- Access to reliable transportation
Preferred Qualifications:
- 2 years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of long-term care, community resources, and cost-effective alternatives
- Knowledge of CPT, ICD-10, and HCPC codes
