Job Summary

SSM Health is seeking a skilled Licensed Practical Nurse to join our Hospice team. As a Nurse Care Specialist, you will provide compassionate care to patients in their homes, working under the direction of a registered nurse or physician.

Key Responsibilities
  • Deliver high-quality care to assigned clients, performing direct care in accordance with established plans of care.
  • Maintain open communication with physicians, supervisors, staff, and family members regarding patient progress.
  • Administer medications and monitor patients for adverse reactions.
  • Document patient care, ensuring accurate and timely reporting.
  • Collaborate with interdisciplinary teams to coordinate patient care and meet regulatory requirements.
Requirements
  • Graduate of an accredited school of nursing or equivalent educational experience.
  • One year of licensed practical nurse experience.
  • Basic Life Support HealthCare Provider (BLS HCP) certification.
  • Licensed Practical Nurse (LPN) licensure in the state of employment.
