Hospice Nurse Job Description

We are seeking a compassionate and skilled Hospice Nurse to join our team at Della Infotech. As a Hospice Nurse, you will provide high-quality patient care and support to individuals and their families during the end-of-life journey.

Key Responsibilities:
  • Provide palliative care to patients and families, utilizing current principles of symptom management and palliative care.
  • Coordinate care in a cost-efficient manner, ensuring the best possible outcomes for patients.
  • Collaborate effectively with the Interdisciplinary Team (IDT) to provide comprehensive care.
  • Utilize the nursing process and current standards of nursing practice to deliver exceptional hospice care.
  • Remain competent in technical nursing skills, including IVs and G-tubes.
  • Provide effective patient and family teaching, promoting independence and self-care.
  • Communicate with the Medical Director, Hospice House staff, and VNA/MVH staff regarding patient status and changes in condition.
  • Understand family dynamics and work effectively within various types of family systems.
  • Demonstrate the ability to act as a team player, providing support and mentoring to peers as needed.
  • Utilize resources appropriately, advocating for patients and families, and providing information about available resources.
  • Accurately identify the need for additional services and communicate patient reports and status changes effectively.
  • Assess patients for fall precautions and update care plans, medication profiles, and HHA treatment plans as necessary.
  • Complete all nursing documentation according to procedures and nursing standards of practice.
  • Manage assignments efficiently and effectively, involving the Team Leader and Medical Director as needed.
  • Maintain professional boundaries with patients and families, utilizing good customer service skills internally and externally.
  • Assist in the orientation of new staff and mentor LPNs and nursing students as requested.
  • Pursue excellence in performance, working with the Team Leader and Hospice House Clinical Manager to identify opportunities for improvement.
  • Participate in achieving MVH goals and objectives, representing the organization and promoting hospice care in the community and among healthcare professionals.
