About the Role

We are seeking a skilled Hospice RN to join our team at High Pointe House in Haverhill, MA. As a Hospice RN, you will provide high-quality nursing care to patients and their families in a variety of settings, including private homes, assisted living facilities, and skilled nursing facilities.

Key Responsibilities
  • Deliver palliative care and symptom management to patients and families
  • Utilize the nursing process and current standards of nursing practice to provide hospice care
  • Remain competent on technical nursing skills, including IVs and G-tubes
  • Understand family dynamics and work effectively within various types of family systems
  • Communicate patient reports and status changes concisely and effectively
  • Assess patient for need for Fall Precautions and update Care Plan, Medication Profile, and HHA Treatment Plan as needed
