Position Overview

We are seeking an experienced and dedicated Healthcare Coordinator to join our ECM team. As a key member of our multi-disciplinary care team, you will be responsible for coordinating all aspects of ECM for our members, ensuring seamless and effective care management and service delivery.



Key Responsibilities
  • Act as the primary point of contact for members, coordinating and managing their care plans, and communicating effectively with care managers and healthcare providers to ensure comprehensive care.
  • Develop, implement, and regularly review care plans to meet the specific needs of each member, leveraging your expertise in healthcare systems and regulations, particularly Medi-Cal benefits and ECM requirements.
  • Collaborate with other care managers and healthcare providers to ensure a cohesive approach to care management, fostering strong relationships and effective communication.
  • Engage with members and their families, providing education and support to promote better health outcomes, and empower individuals to take control of their healthcare journey.

Requirements
  • A minimum of [Estimated Salary: $80,000] per year salary, offering competitive compensation and benefits package.
  • Significant experience in care management, preferably in a community-based setting, with a proven track record of delivering high-quality care and achieving positive outcomes.
  • Strong interpersonal and communication skills, with the ability to work effectively as part of a multi-disciplinary team, and build strong relationships with members, caregivers, and healthcare professionals.
  • Knowledge of healthcare systems and regulations, particularly Medi-Cal benefits and ECM requirements, with the ability to stay up-to-date with changes and developments in the field.
  • Empathy and commitment to person-centered care, with a passion for delivering compassionate and patient-focused care.
