Pharmacist - Long-Term Care Specialist

About the Role:
We are seeking a highly skilled Pharmacist to join our team at Res-Care, Inc. as a Long-Term Care Specialist. In this role, you will be responsible for evaluating physician medication orders, maintaining patient/resident databases, and collaborating with the healthcare team to provide clinical and technical support.
Key Responsibilities:
- Evaluate physician medication orders for appropriateness and make recommendations for therapy changes as needed.
- Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
- Collaborate with the healthcare team to provide clinical and technical support, including ordering and evaluating laboratory tests for monitoring high-risk conditions.
- Document all communication with physicians, nurses, and other members of the healthcare team in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
Requirements:
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
