Hospice Nurse Specialist

HealthCare Support is seeking a skilled Hospice Nurse Specialist to join their team in Sacramento, CA. As a Hospice RN, you will be responsible for providing high-quality patient care and support to individuals in need of end-of-life care.
Daily Responsibilities:
- Complete daily visits to patients to assess their needs and provide education
- Collaborate with the medical team to maintain care plans and make adjustments as needed
- Communicate with patients and families on end-of-life processes and provide physical and emotional support
Required Qualifications:
- Active RN license in California
- 1+ year of experience as an RN and recent hospice experience
