Hospice Nurse Case Manager Job Description

We are seeking a compassionate and skilled Hospice Nurse Case Manager to join our team at Insight Global. As a Hospice Nurse Case Manager, you will play a critical role in providing high-quality care to patients and their families during challenging times.

Key Responsibilities:
  • Assess patients and their families for physical, psychological, social, and spiritual needs.
  • Collaborate with patients, families, physicians, and the interdisciplinary group (IDG) to establish personalized care plans.
  • Implement interventions and evaluate care effectiveness.
  • Document patient information according to established protocols.
  • Participate in on-call duties and admissions assignments as directed.
  • Provide education to patients and caregivers based on their specific knowledge needs and learning assessments.
  • Address patient and family conditions with empathy and expertise.
  • Coordinating care with the IDG.
  • Inform the Team Director and Team Manager of any unusual or potentially problematic patient/family issues.
  • Participate in IDG meetings, facility patient conferences, and other patient/family meetings as coordinated with the IDG and/or Team Director.
Requirements:
  • AA or Bachelor of Science in Nursing
  • Registered Nurse License in California
  • BLS - Basic Life Support
  • CPR
  • Hospice experience for 6 months to a year
  • EMR Care Point (homecare/homebase/point care) Case Manager within Hospice
