Position Overview

  • Conduct initial and ongoing thorough evaluations of the effects of terminal illnesses on patients' physical, functional, psychosocial, and environmental requirements, as documented in clinical records and team reports.
  • Utilize specific criteria for patient admission and re-certification to hospice services, ensuring appropriate care levels and patient eligibility.
  • Develop, implement, and document comprehensive care plans to maintain quality and continuity of care, adjusting plans as necessary.
  • Educate patients and their families about disease processes, self-care strategies, end-of-life care, and nutritional needs, while collaborating with interdisciplinary team members.
  • Assess the scope and frequency of required services based on patient acuity and family needs, evaluating caregivers' capabilities to meet immediate patient requirements.
  • Initiate preventive and rehabilitative nursing interventions as needed.
  • Apply infection control principles and standard precautions in all patient care activities to minimize contamination risks.
  • Provide clinical guidance to Hospice Aides and LPNs/LVNs to ensure consistent and high-quality service delivery.
  • Prepare clinical documentation and progress notes that reflect advancements toward established care goals, ensuring quality patient care is consistently documented.
  • Communicate with physicians and team members regarding any changes in patient needs and the outcomes of interventions.
Qualifications

Required Experience and Skills

Graduation from an accredited nursing program (Diploma, Associate, or Baccalaureate).
  • Licensure: Must hold a current state RN license and/or comply with Nurse Licensure Compact regulations. A valid driver's license is also required.
  • Experience: Previous experience as a registered nurse in a clinical setting is essential; hospice experience is preferred.
  • Skills: Proficient nursing skills aligned with accepted standards of practice, strong interpersonal abilities, and specialized skills in palliative and end-of-life care are advantageous. Proficiency in reading, writing, and understanding English is required.
Transportation: Reliable transportation and valid auto liability insurance are necessary.

Compensation: $75-$150 per visit
