About the Position:

We are seeking a skilled Nurse Practitioner to join our team in a PRN/Part Time capacity. As a Nurse Practitioner with Matrix Medical Network, you will conduct home risk assessments, review medical history, and provide health education to our members. You will work collaboratively with physicians, case managers, and other healthcare professionals to deliver comprehensive care and improve patient outcomes.

Responsibilities:

* Conduct adult/geriatric assessments and medical history reviews
* Provide health education and counseling to patients and their families
* Collaborate with physicians and other healthcare professionals to develop and implement care plans
* Conduct home visits and provide care in a variety of settings, including skilled facilities and telehealth

Requirements:

* Master's degree in nursing or related field
* Current RN and NP licensure in state of practice
* Board certification in a Matrix-approved specialty
* 1 year of experience as a Nurse Practitioner
* Strong computer skills and familiarity with electronic health records
