About the Role:
  • Develop and implement strategies to improve patient outcomes and reduce healthcare costs.
  • Supervise and develop a team of RNs, SWs, and non-clinical support staff to ensure effective case management services.
  • Evaluate and maintain case management skills and outcomes, ensuring adherence to key model components.
  • Collaborate with the multidisciplinary team to provide quality, timely, and cost-efficient care.
Requirements:
  • Minimum 7 years acute clinical experience.
  • RN with Master's Degree.
  • Current License as Registered Nurse in the Commonwealth of Massachusetts.
  • Strong analytical and problem-solving skills, with ability to communicate complex information effectively.
  • Administrative skills to supervise and develop staff, plan, organize, direct, and analyze data.
  • Strong understanding of clinical and financial outcomes, with ability to interact effectively with diverse personalities and cultural backgrounds.
Preferred Qualifications:
  • Minimum 5+ years of progressive management experience with demonstrated goal-oriented outcomes.
  • Leadership and change management experience, with ability to work in a fast-paced environment.
