Job Title: Medical Team Coordinator

We are seeking a highly skilled and organized Medical Team Coordinator to join our team at Edison Home Health Care. As a key member of our nursing team, the Medical Team Coordinator will work closely with registered nurses to ensure seamless patient care.

Responsibilities:
  • Initiate daily reports and start-of-care tasks
  • Run daily discharge reports and weekly reassessment reports
  • Assign registered nurses to complete visits and follow up on paperwork
  • Maintain accurate records in patient folders and exchange systems
  • Coordinate incident visits and supervise home health aides
Requirements:
  • High school degree or equivalent required
  • Experience in healthcare/home care preferred
  • Knowledge of HHA Exchange and proficiency in Microsoft Office
  • Excellent communication and problem-solving skills
