Job Title: Registered Nurse

We are seeking a skilled and compassionate Registered Nurse to join our team at the VNA of the Treasure Coast. As an Admissions RN, you will play a critical role in providing high-quality patient care and ensuring the seamless transition of patients into our home health services.

Responsibilities:
  • Conduct comprehensive admission assessments, including the completion of the OASIS and primary and secondary diagnoses, to develop a personalized plan of care.
  • Coordinate and collaborate with the interdisciplinary team to ensure the initial plan of treatment meets the patient's needs.
  • Communicate the recommended plan of care to the patient's physician and provide ongoing care coordination.
  • Perform patient follow-up visits, provide care in accordance with approved physician orders, and document clinical findings and interventions.
  • Stay up-to-date with Medicare regulations and coverage guidelines, as well as other regulatory agencies' rules governing patient care.
Requirements:
  • Current RN licensure in the state of Florida.
  • Minimum of one year of full-time experience in a Medicare home health environment.
  • Proficiency in OASIS.
