Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice RN Case Manager. In this role, you will be responsible for providing high-quality patient care, collaborating with interdisciplinary teams, and ensuring seamless coordination of services.

Key Responsibilities:
  • Provide direct patient care services, including assessments, care planning, and implementation
  • Collaborate with physicians, family members, and other healthcare professionals to develop and implement patient care plans
  • Manage patient records, ensuring accurate and timely documentation
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices
Requirements:
  • RN license in the state of employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, or related settings
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Ability to work independently, with minimal supervision, and as part of a collaborative team
