Job Summary
We are seeking a Certified Patient Care Coordinator to join our team. This role will be responsible for providing direct patient care, assisting with daily tasks, and ensuring that all patient needs are met.

Responsibilities
- Assist patients with daily living activities such as bathing, dressing, and grooming
- Provide emotional support and companionship to patients
- Assist with mealtime and other daily tasks
- Maintain accurate records of patient care and progress
- Collaborate with healthcare teams to develop and implement patient care plans

Requirements
- Must have a strong commitment to providing excellent patient care
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Excellent communication and interpersonal skills
- Reliable transportation to travel to patient homes and facilities

Benefits
