Palliative Medicine Specialist

We are seeking a highly skilled Palliative Medicine Specialist to join our team at the Greater Baltimore Medical Center (GBMC). As a Palliative Medicine Specialist, you will work closely with patients, families, and healthcare teams to provide comprehensive care and support.
Key Responsibilities:- Develop and implement individualized care plans to address the physical, emotional, and spiritual needs of patients and families.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Provide education and support to patients and families on palliative care options and resources.
- Participate in quality improvement initiatives to enhance palliative care services.
- Master's degree in Nursing or related field.
- Current registration with the Maryland State Board of Examiners of Nurses as a Nurse Practitioner.
- At least 5 years of experience in a healthcare environment, with at least 1 year in palliative care.
- Strong analytical and communication skills, with ability to work effectively in a team environment.
- Certification in Hospice and Palliative Care through the Hospice and Palliative Credentialing Center (HPCC).
- Experience with electronic medical records and other healthcare technologies.
