Key Responsibilities:

  • Respond to patient inquiries in a professional and empathetic manner
  • Coordinate medical specialty referrals and procedures efficiently
  • Utilize electronic health records (EHRs) to track and research urgent requests
  • Communicate information effectively through various channels
  • Review patient charts and records for necessary authorizations
  • Ensure all barriers to care are addressed
  • Document interactions accurately in EHRs
  • Process prior authorizations and insurance referrals
  • Maintain compliance with HIPAA and patient confidentiality standards
  • Utilize related systems to coordinate care and update patient records
  • Proactively identify patient care needs
  • Navigate patients to assigned care
