Job Title: Habilitation Specialist

Goodwill NYNJ is seeking a skilled Habilitation Specialist to join our team. In this role, you will work with individuals with intellectual disabilities to coordinate pre-vocational and day habilitation services, engage in person-centered planning, and implement services and activities to promote independence.

Key Responsibilities:
  • Coordinate pre-vocational and day habilitation services for a caseload of up to 30 individuals with intellectual disabilities.
  • Engage each individual in a person-centered planning process to identify their goals and implement services and activities to achieve their highest level of independence.
  • Complete all required documentation efficiently and to the standards of funding sources and oversight agencies.
  • Coordinate group and individual scheduling of activities and services, and regularly visit program and volunteer sites.
  • Oversee the implementation of services by direct support professionals and write and update individual habilitation plans and person-centered plans.
Requirements:
  • Bachelor's degree from an accredited school in a related field.
  • One year of work experience in mental health, vocational rehabilitation, or case management preferred.
  • Strong interpersonal and communication skills, with the ability to work independently with minimal supervision.
  • Flexibility and openness to new ideas, with a solution-oriented approach to problem-solving.
Working Conditions:
  • Frequent travel and field work within the boroughs.
  • Attendance at individual meetings that may occur in the evenings and/or at off-site locations.
