Patient Care Coordinator

The Patient Advocate plays a vital role in the clinical healthcare team at CareSouth Carolina. Key responsibilities include:
Key Responsibilities- Assisting patients in obtaining prescribed medications, including communication with the CSC Pharmacy and completion of Prescription Assistance Program applications.
- Collaborating with staff to improve patient care and outcomes, with a focus on providing high-quality, patient-centered care.
- Participating in trainings, team meetings, and huddles to facilitate the delivery of care.
- Referring patients to facilities outside of CSC as needed.
- High school diploma or equivalent required.
- Completion of a certified medical program, such as MOA, CMA, or CNA, preferred.
- One year of medical office or community health center experience preferred.
