Your expertise will be essential in monitoring, evaluating, and managing the care given to residents to ensure compliance with federal, state, and ethical standards.

Your responsibilities will include continuously evaluating residents to determine if goals are reached and if the resident remains eligible for their current services or changes in the care plan. You will also screen all potential admissions for Medicare/Medicaid eligibility, coordinate and conduct Welcome care plan meetings for new admissions, and work with the Business Office and Rehabilitation Department to validate reimbursements from Medicare, Medicaid, VA, or other entities.

As a member of our interdisciplinary assessment and care planning team, you will serve as chairperson/nursing coordinator to discuss goals and plans of action and inform the resident and/or family or responsible party of goals and plans of action. You will work closely with nursing and therapy departments to coordinate Medicare skilled services and conduct weekly meetings with all Healthcare staff regarding rehabilitation residents for progress and next steps.

You will also ensure accuracy of medical records and coding procedures, prepare and provide reports to regulatory agencies and the Director of Nursing in a timely manner as required, and submit MDS data to the CMS website timely as required. Additionally, you will prepare a quarterly quality indicator report for the Director of Nursing using data from the CMS site to monitor alerts and needed action.

To be successful in this role, you will need to maintain up-to-date knowledge of federal and state regulations, particularly Medicare and Medicaid programs, and ensure compliance with all regulations from DHEC, CMS, Fire and Safety, OSHA, Labor Laws, etc. and adhere to HIPAA confidentiality standards.
