We are seeking a highly skilled RN Care Manager to join our Behavioral Health Interdisciplinary Program team. As a RN Care Manager, you will be responsible for providing high-quality patient care and coordinating with interdisciplinary teams to ensure seamless care delivery.
Key Responsibilities- Assess, plan, implement, and evaluate care based on Mental Health (MH) specific components.
- Coordinate care focused on patient education, self-management, and customer satisfaction throughout the MH continuum of care.
- Follow established policies and guidelines, and collaborate with the interdisciplinary team to influence care outcomes.
- Provide competent, evidence-based care to assigned patients, and assist in directing the provision of nursing education and quality improvement initiatives.
- Facilitate warm hand-offs, tele mental health care, and nurse-led psychoeducational groups, and utilize therapeutic communication techniques to promote patient-centered care.
- Participate in care coordination, liaise with Inpatient MH Units, and respond to psychiatric and medical emergencies as directed.
- Report and address safety concerns, document nursing triage assessments, risk screens, clinical reminders, and patient plan of care, according to facility policy.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
- 3-5 years of Behavioral Health experience.
- 3-5 years of more of outpatient primary care.
- 3-5 years work experience with adults.
- 3-5 years preferred BHIP experience.
- Competitive salary and regular salary increases.
- Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
- Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA.

- Licensure: 1 full and unrestricted license from any US State or territory.
- Work Schedule: Compressed Tuesday - Friday.
