Clinical Care Coordinator

As a Registered Nurse at St. Peter's Health, you will be responsible for providing direct patient care in various settings, including the home and general inpatient environments. You will work under the direction of the Director or designee and perform detailed and accurate patient assessments utilizing a comprehensive assessment.
The RN will complete documentation in a timely and accurate manner according to hospice policy and regulatory requirements. You will facilitate patient hand-offs and provide assistance to move the patient through life transitions. Additionally, you will understand the principles of case management to ensure effective coordination of care and utilization of community resources.
Key responsibilities include:
- Providing direct patient care in the home and general inpatient settings
- Performing detailed and accurate patient assessments
- Completing documentation according to hospice policy and regulatory requirements
- Facilitating patient hand-offs and providing assistance with life transitions
- Understanding the principles of case management
Communication:
You will demonstrate good communication skills to facilitate implementation of quality core measures, attend meetings and weekly IDT case conferences. You will maintain open communication with the interdisciplinary team and the Director and/or clinical coordinator in relation to case management responsibilities or patient concerns.
Adaptability:
You will have a 'can do' attitude, keep the department running smoothly, promote teamwork, and a no-blaming environment while setting the tone for the department by being a good role model. You will maintain a positive attitude and flexibility regarding fluctuating patient census and staffing needs.
