We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to patients with tracheostomy and ventilator needs in the comfort of their own homes.

Job Summary:

The successful candidate will be responsible for delivering high-quality patient care, documenting patient progress, and collaborating with healthcare teams to ensure optimal patient outcomes.

Key Responsibilities:
  • Provide skilled nursing care to patients with tracheostomy and ventilator needs
  • Document patient progress and communicate with healthcare teams
  • Collaborate with patients and families to develop and implement individualized care plans
  • Participate in ongoing education and training to stay current with best practices and industry developments
Requirements:
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
Physical Requirements:
  • Must be able to speak, write, read, and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must be able to sufficiently reposition patients and move equipment without assistance
Environment:
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids, and infectious diseases
