Responsibilities:- Provide direct patient care, including administering medications, performing wound care, and documenting patient information.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Conduct resident rounds, direct care for certified nursing assistants (CNAs), and report problems to the nursing supervisor.
- Prepare and assign nursing assistant assignments and duties, and supervise all direct resident care.
- Perform data collection, work with the registered nurse to develop an accurate comprehensive care plan, and participate in resident care conferences.
- Counsel nursing assistants and recommend disciplinary action when necessary, and participate in the employee evaluation process.
- Other clinical duties include checking emergency equipment, identifying safety hazards, and notifying physicians of changes in patient conditions.
- Graduated from an approved Practical Nurses Education Program.
- At least one year of work experience in rehabilitative, psychiatric, or geriatric nursing preferred.
- A Licensed Practical Nurse licensed in the State of Connecticut.
- Excellent communication and interpersonal skills.
