Veteran Care Coordinator-RN IV Position

Job Summary:

The Massachusetts Veterans Homes are seeking a skilled Veteran Care Coordinator-RN IV to lead our team in providing exceptional care to our veteran residents. As a key member of our nursing leadership team, you will be responsible for managing designated Veteran Care Centers, ensuring high-quality care, and promoting a positive work environment.

Key Responsibilities:

Assess, plan, and evaluate nursing care and veteran progress towards goals, directing staff as needed.

Ensure timely and accurate completion of the Resident Assessment Instrument (RAI) process, including MOS/RAPS.

Participate in interdisciplinary team processes to integrate nursing into a coordinated plan of care with all other support services.

Maintain own competence and act as a clinical resource and expert role model to staff.

Hold staff accountable for their clinical practice, including use of the nursing process and appropriate documentation to meet regulatory body standards.

Requirements:

Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.

CPR certification.

At least two years of full-time, or equivalent part-time, experience as a registered nurse in a recognized hospital, clinic, or medical facility, with at least one year in a supervisory, administrative, or managerial capacity.
