Job Title: Registered Nurse - Case Management

Mass General Brigham is seeking a skilled Registered Nurse (RN) to join our Case Management team in Chelsea, Massachusetts.

Job Summary: We are looking for a dedicated and compassionate RN to collaborate with our healthcare team to enhance the delivery of patient care services along the continuum of care.

Responsibilities:

  • Evaluate and predict patient care needs to ensure efficient and expeditious delivery of services
  • Collaborate with the healthcare team to develop and implement individualized care plans
  • Coordinate and facilitate patient care services to ensure seamless transitions and optimal outcomes
  • Monitor and evaluate patient care services to identify areas for improvement and implement changes as needed
  • Advocate for patients and families to ensure their needs are met and their voices are heard

Requirements:

  • Current RN licensure in the state of Massachusetts
  • Minimum of 2 years Med/Surg experience
  • Case Manager certificate preferred
  • ADN required, but BSN strongly preferred
  • Acute hospital experience preferred
