Job Summary

We are seeking a highly motivated and compassionate Care Coordinator to join our Behavioral Health Community Partner (BH CP) program. As a key member of our team, you will play a vital role in supporting the health, safety, and quality of life of our clients through advocacy, teaching, and service planning.

Key Responsibilities
  • Provide outreach and engagement with enrollees using telehealth and/or community visits
  • Assist enrollees to identify goals and develop an individual care plan
  • Complete comprehensive assessments
  • Facilitate care teams
  • Coordinate communication among enrollees, their providers, involved state agencies, and others
  • Support enrollees to access and utilize resources to improve and maintain their health and quality of life
Requirements
  • Bachelor's degree in a related field
  • Focus in psychology highly preferred
  • 1-2 years of human service experience with a similar population preferred
  • Valid driver's license and access to a reliable vehicle required
  • Bilingual skills highly needed - please indicate language skills in application
