About the Role:

We are seeking a highly qualified Licensed Clinical Social Worker (LCSW) to join our team in Pittsburgh, PA. As a Mental Health Care Coordinator, you will be responsible for coordinating and supporting mental health care within the clinic and for coordinating referrals to clinically indicated services outside the clinic.

Key Responsibilities:

  • Screen and assess patients for common mental health and substance abuse disorders
  • Provide clinically appropriate treatment based on an objective mental health assessment
  • Develop and implement treatment plans in collaboration with healthcare providers

Requirements:

  • Master's degree in social work with a minimum of 2 years' experience
  • LCSW Certification required (PA)
  • At least 2 years clinical social work experience in community healthcare environment
  • Clearances: FBI, State Criminal Records Check, Child Abuse Clearance
  • Valid driver's license and reliable transportation
