Key Responsibilities- Deliver post-acute care to patients, including monitoring their physical and mental wellbeing and tracking vital signs.
- Perform intakes of patients, documenting their health concerns and medical histories, and reviewing medical records and charts.
- Administer medications, verify their accuracy, and maintain appropriate levels of medical supplies.
- Contribute to a safe and comfortable environment for patients, assisting with IV set up, vaccinations, and phlebotomies.
- Be accountable for following and adhering to care plans for patients and helping medical staff with orientation and training.
- Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN).
- Completion of an approved Licensed Practical Nurse (LPN) educational program.
- High school diploma (or GED/equivalent).
- Minimum of 6 months of experience as an LPN or in a related role.
- Ability to lift a minimum of 50 lbs.
- Competitive hourly pay range of $32-$65.
