Job Title: Licensed Practical Nurse

Sunrise Senior Living is seeking a highly skilled and compassionate Licensed Practical Nurse to join our team. As a Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents.

Responsibilities:
  • Conduct monthly wellness visits for all residents to identify on-going needs and services.
  • Assist the Resident Care Director in conducting resident evaluations and gathering information at various stages.
  • Monitor and follow up with residents who have experienced falls, accidents, or incidents, documenting and reporting as required.
  • Communicate effectively with residents' families and responsible parties to ensure medical needs are met.
  • Collaborate with the Resident Care Director and community team to promote infection control standards and quality care.
Qualifications:
  • Graduate of an approved college or school of nursing.
  • Current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  • Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
  • Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
  • Ability to handle multiple priorities and possess excellent communication and problem-solving skills.
