Job Summary

The Value-Based Care Management Registered Nurse is a key member of our team, responsible for providing comprehensive care to patients with chronic kidney disease. This role involves interacting with patients, families, and support networks to educate and support them in managing their condition.

Key Responsibilities
  • Assess patient needs and develop personalized care plans
  • Provide health education and treatment decision support to patients and their families
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Conduct regular patient visits and outbound calls to monitor progress and identify areas for improvement
  • Work closely with nephrologists and other healthcare professionals to support quality care
  • Assist with early identification and support for dialysis and transplant needs
  • Provide patient education to promote self-management and improve health outcomes
Requirements
  • Current RN licensure and relevant experience in nephrology or related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of federal, state, and local laws and regulations governing healthcare
