Job Title: Home Health Speech Therapist

Alternate Solutions Health Network LLC is seeking a skilled Home Health Speech Therapist to join our team in Detroit, Michigan.

Job Summary:

We are looking for a compassionate and experienced Speech Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Speech Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that address the unique needs of each patient.

Responsibilities:
  • Develop and implement individualized plans of care for patients with speech and language disorders.
  • Provide one-on-one therapy sessions to patients in their homes, focusing on improving communication skills, swallowing, and breathing functions.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordinate services.
  • Document patient progress and outcomes, and communicate effectively with patients, families, and healthcare teams.
  • Stay up-to-date with industry developments and best practices in speech therapy.
Requirements:
  • Current licensure as a Speech Therapist in the state of Michigan.
  • Minimum one year of experience as a Speech Therapist in a home health care setting.
  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
