About the Role:
  • Provide home-based skilled speech therapy care for patients as directed by the attending physician.
  • Develop individualized ST plans of care and make adjustments as needs change.
  • Guide patients and their family members in the use of therapeutic, creative, and self-care activities to improve functioning.
  • Observe, record, and report patient reactions, changes, and progress to attending physicians and agency staff.
Responsibilities:
  • Plan of Care: Develop and implement individualized ST plans of care, making adjustments as needed.
  • Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication.
  • Minimize Patient Risk: Teach patients and their families on preventative measures to reduce re-hospitalizations.
  • Family Educator/Advocate: Counsel patients and families on disease management and provide education on speech therapy-related functions.
  • Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
Requirements:
  • Current license as a Speech Therapist in the state of employment.
  • Minimum one-year experience as a ST in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Ability to perform all physical demands of the job.
