Speech Language Pathologist Opportunity

We are seeking a skilled Speech Language Pathologist to join our dynamic team at Hope at Home Health Care. As a Speech Language Pathologist, you will provide speech therapy services to patients in their homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities:
  • Assess and evaluate patients' rehabilitative and functional status, and participate in the development of their total plan of care.
  • Provide speech therapy services as ordered by a physician, including assessing and evaluating home environments.
  • Improve or maximize patients' communication abilities.
  • Assist in developing and reviewing care plans, and prepare and submit clinical and progress summaries based on patients' goal attainment.
  • Participate in discharge planning for patients, and prepare and submit clinical progress summaries based on patients' goal attainment.
  • When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.

Requirements:
  • A Master's degree from a Speech-Language Pathology program certified by the American Speech Association.
  • A license and certification as a Speech-Language Pathologist, issued by the state of Michigan.
  • A minimum of one year of healthcare experience.
