Job Title: Speech Language Pathologist for Home Health

Alternate Solutions Health Network LLC is seeking a skilled Speech Language Pathologist to join our team in Detroit, Michigan.

About the Role:

We are looking for a compassionate and experienced Speech Therapist to provide home-based skilled speech therapy care for patients. As a Speech Language Pathologist, you will work with patients and their families to develop individualized plans of care and make adjustments as needed.

Responsibilities:
  • Develop and implement individualized plans of care for patients
  • Provide home-based skilled speech therapy care for patients
  • Educate patients and their families on therapeutic activities and self-care
  • Observe, record, and report patient reactions, changes, and progress to attending physicians and agency staff
  • Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication
  • Minimize patient risk by teaching preventative measures and making good decisions
  • Counsel patient and family on disease/injury and how to manage in all phases of speech therapy
Requirements:
  • Speech Therapist with a current license in the state of employment
  • Minimum one-year experience as a ST in an acute care setting. Home care experience preferred
  • Valid driver's license and auto insurance in your name as a driver
  • Capable of all physical demands
