Mental Health Resource Center is a non-profit organization dedicated to providing a comprehensive range of mental health and behavioral health services to the community. Position Summary
The Care Coordinator within our Behavioral Health Care Management program plays a crucial role in assisting individuals who are not effectively connected to the necessary services and supports. This position primarily focuses on uninsured or underinsured individuals who frequently utilize acute care services, including crisis stabilization, inpatient care, and detoxification. The Care Coordinator will evaluate individual needs, develop a coordinated care plan, and conduct outreach to engage individuals referred from various sources.


Key Responsibilities

  • Accountability: Serve as the primary point of contact for coordinating services and supports, ensuring a holistic approach to meet individual needs.
  • Engagement: Establish trust and rapport with individuals by actively reaching out and involving their natural support systems in the care plan.
  • Assessment: Utilize standardized tools to determine the appropriate level of care.
  • Collaborative Planning: Develop individualized, strength-based care plans that prioritize the individual's values and preferences.
  • Community Integration: Ensure services are delivered in inclusive and accessible settings that promote community involvement.
  • Cross-System Coordination: Integrate services across various sectors, including physical health, behavioral health, and social services.
  • Information Sharing: Comply with legal standards to share information among service providers and support systems.
  • Effective Transitions: Facilitate introductions between individuals and their care coordinators to ensure continuity of care.
  • Cultural Competence: Respect and incorporate the individual's cultural values and beliefs into the care process.
  • Outcome Monitoring: Align care plan goals with measurable success indicators and adjust plans as necessary.
  • Symptom Stabilization: Assist in stabilizing mental health symptoms through effective care coordination.
  • Advocacy: Advocate for necessary services and resources to implement care plans, ensuring effective service delivery.
  • Outreach: Conduct outreach to individuals referred from various sources, providing information about available services.
  • Regular Follow-Up: Maintain ongoing contact with individuals to ensure they remain connected to services.
  • Provider Outreach: Engage with service providers to inform them about available resources.

Qualifications

  • A Bachelor's Degree in Social Work or a related Human Services field from an accredited institution is required.
  • At least one year of experience in human services or a mental health-related field is required, with a preference for experience working with adults with mental illness.
  • Proficiency in electronic health records and patient information systems is expected within three months of employment.
  • Strong skills in Microsoft Office and internet usage are required.
  • Must possess a valid driver's license and meet frequent driver requirements.
  • Ability to travel to various locations for outreach and home visits is necessary.
  • Excellent communication skills are essential for effective interaction with diverse stakeholders.

Position Details
