The Family Support Specialist plays a crucial role in orchestrating a thorough case management strategy for youth and families, ensuring the safety, stability, and overall well-being of children in foster care.

This position is primarily community-based, requiring travel across various counties.

Key Responsibilities


1. Conduct assessments of family strengths and needs, identifying problem areas and employing diverse treatment methods and resources to foster growth and positive change.

2. Engage in the development, execution, and continuous evaluation of a comprehensive treatment and discharge plan.

3. Create alternative placement or permanency options for youth who may not be returning to their families, ensuring a detailed discharge summary is completed well in advance of discharge.


4. Provide effective case management services, including scheduling necessary counseling sessions and ensuring children receive required medical and dental care.

5. Monitor and document services provided, ensuring all records are completed in a timely and thorough manner.

6. Uphold confidentiality standards, securing parental consent and necessary information releases.


7. Conduct regular casework contacts with families, focusing on treatment goals through home visits and community interactions.

8. Build strong, supportive relationships with youth and families, ensuring children are placed in safe and nurturing environments.


9. Serve as a liaison between youth, families, and foster care treatment teams through various communication methods, including phone calls, in-person meetings, and treatment reviews.

10. Participate in treatment review meetings.


11. Assist in the reunification of youth with their families and communities by implementing permanent support systems, such as mentoring, after-school activities, and relapse prevention programs.


Location

The office location for this position is designated, with travel required to the office several times a month.

Qualifications

1. A minimum of a Bachelor's degree in human services or a related field is required.

2. Preferred experience includes at least one year working with at-risk youth, families, and children aged 0-21 in the social work sector.


3. Flexibility in scheduling is essential, with variable hours including days, evenings, weekends, and holidays.

4. A valid driver's license and reliable transportation are necessary, along with the ability to efficiently coordinate travel for visits and appointments.

5. Experience working with a diverse population is highly valued.

Skills


1. Proficiency in crisis intervention and solution-focused approaches.

2. Strong communication skills, encompassing written, verbal, and listening abilities.

Salary Range

Compensation may vary based on skills, experience, and other relevant factors.

1. $25.24 per hour ($52,400 annually) - Bachelor's degree with no relevant experience.

2. $26.20 per hour ($54,400 annually) - Bachelor's degree with two years of relevant experience.
