Job Summary

We are looking for a highly motivated Patient Support Navigator to join our team. As a key member of our MediCal program, you will play a vital role in supporting members in improving their overall health through outreach and engagement activities.

The successful candidate will navigate the healthcare system, advocate for members, and collaborate with medical teams to ensure seamless care delivery. You will also be responsible for providing recurring care coordination for members, facilitating referrals to community support services, and coordinating care plans with patients' primary care physicians (PCPs) and other healthcare providers.

Key Skills and Qualifications:

  • Proven experience in patient advocacy, community outreach, or a related field; Excellent communication and interpersonal skills; Ability to work effectively with diverse populations, including vulnerable and underserved communities; Strong organizational and time management skills; Proficiency in Microsoft Office and Google Suite; Familiarity with electronic health records (EHRs); Willingness to work Monday-Friday 8:30am-5:00pm, primarily in the field.
  • Bilingual in Spanish preferred; CHW Certification or ability to complete certification within 6 months of employment; Valid California Driver's license, valid automobile insurance, and a clean driving record; Familiarity with local community resources and services; Ability to work independently and collaboratively as part of a team.
