We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at ComForCare Home Health Care - Southern Middlesex County. As an LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.
Job Summary:The LPN will be responsible for delivering skilled nursing functions in accordance with federal, state, and local laws, as well as the guidelines of their professional organization and Agency. This includes performing tasks such as administering medications, wound care, and vital sign monitoring.
Responsibilities:- Deliver skilled nursing functions in accordance with established policies and practices, as well as state Nurse Practice Act.
- Provide direct care to clients as delegated by the case managing RN or supervisor.
- Collaborate with the healthcare team to ensure continuity of care for patients.
- Document patient care and progress in accordance with Agency policies.
- Participate in ongoing education and training to maintain current knowledge and skills.
- Graduate of an accredited school of professional nursing.
- Current LPN license required.
- Minimum of one (1) year experience in an acute care setting or equivalent experience.
- CPR certified.
- Demonstrated written, verbal, and interpersonal communication skills.
- Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair.
