Care Coordinator Professional

At Belvedere Home Care, we are seeking a highly skilled Care Coordinator Professional to join our team. This is a full-time position that requires travel within the Capital District.
About the Role:This role involves providing exceptional care coordination services to individuals with disabilities and seniors. As a Care Coordinator Professional, you will be responsible for maintaining active caseloads, reviewing and updating Service Plans, conducting monthly face-to-face meetings, and ensuring timely completion of service plans.
The ideal candidate will have a strong understanding of case management principles, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Responsibilities:- Maintain accurate and up-to-date records of waiver transportation and participant information.
- Collaborate with waiver service providers to ensure seamless delivery of services.
- Facilitate team meetings and provide regular updates on participant progress.
- Conduct home visits and community-based assessments as required.
- Develop and implement individualized service plans that meet the unique needs of each participant.
To be considered for this role, you must possess a degree in a relevant field (e.g., social work, psychology, gerontology) and have at least one year of experience in case management or a related field. You should also have excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
We offer a competitive salary range of $18.00 to $20.00 per hour, depending on experience, as well as comprehensive benefits, including medical, dental, vision, and life insurance.
