About the Role

As a Health and Wellness Nurse, you will be responsible for:

  • Providing high-quality, patient-centered care to our residents
  • Developing and implementing individualized care plans to meet the unique needs of each resident
  • Collaborating with our interdisciplinary team to ensure seamless care coordination
  • Providing education and support to residents and their families on health and wellness topics

Requirements

To be successful in this role, you will need:
